I consent to VACCIPACK app showing me health promotion messages in the form of stories, questions from users and responses to questions from users and experts, links to web resources, my safety log and my vaccine tracker and sending me push notifications for reminders for upcoming appointments via my mobile phone. I understand that VACCIPACK uses the minimum information necessary in the push notification to reduce the chance of someone else seeing my health information. I understand that VACCIPACK is not 100% private and secure, which means that other people might be able see the app contents, including the push notifications that are intended to be private. I also understand that the VACCIPACK uses electronic storage "in the cloud." While the system uses some encryption and auditing and other protections, the cloud services are not 100% private and secure. I understand that the system's security may be breached, which means that information about me could be accessed by unauthorized persons and could be made public. I am interested in receiving the push notification in any event, as
part of the VACCIPACK app.
VACCIPACK is a proof of concept prototype actively being developed. For that reason the development team may collect my usage data. I consent to questions and responses I post to the discussion forum can be viewed by other users and my personal information (while not seen by other users), but can be viewed by the development team.
